Please submit this form as soon as possible after the incident has occurred. It will be submitted to our Safety Officer who shall in turn forward it on to Paddle UK where applicable.

Data Protection

British Canoeing is a privacy conscious organisation and is strongly committed to an individual’s right to privacy. All data gathered following the completion of this form will only be utilised to administer the incident report. Any personal data will be processed and stored in compliance with the British Canoeing Data Protection Policy and in accordance with all applicable Data Protection laws in effect at the time of publication of this form, including but not limited to, the Data Protection Act 2018 and the GDPR which came into effect on 25th May 2018. Data may be shared securely with statutory authorities for the purposes of investigating the incident, or with other third parties where we are legally required to do so. In all other circumstances, data will be anonymised before sharing.
